*Pre-existing conditions means that any medical conditions you have for up to 12 months prior to your effective date of coverage with this plan are not covered for the benefits listed above for 12 months following your effective date of coverage. All benefits, as described in your certificate of coverage, begin 30 days following the effective date of your coverage, except injury which begins mmediately.

Availability
The Apple Plans are limited benefit indemnity plans available in the following states:
Alabama, Delaware, Georgia, Indiana, Kentucky, Missouri, Mississippi, Montana, New Mexico, Nevada, Pennsylvania, South Dakota, Wisconsin.
Eligibility Information
The Group Indemnity Insurance Plan, underwritten by American Financial Security Life Insurance Company, is provided to eligible members of National Congress of Employers (NCE) Association, U.S. Citizens, who are under age 64 years and 6 months and not Medicare eligible. In South Dakota, association membership is optional and coverage is provided by individual policy of insurance form AFFI-IND-POL-SD 410. Eligible dependents are the spouse and each unmarried child of the Member up to age 25 (up to age 29 in SD for full-time students with no creditable coverage). Coverage cannot be issued to a child only (under age 18).
